ID-UTI Flashcards
In most cases what does candiduria mean?
It usually means colonization and does not require treatment. If symptomatic and no other causes in patients with foleys or diabetes, then can treat.
Why is antimicrobial resistance occurring amongst urinary pathogens?
Antibiotic overuse
Inappropriate treatment of asymptomatic bacteuria
local differences in antibiotic use
What are the two reasons for treating asymptomatic bactiuria?
Pregnancy
Expected urological procedure
What are good first line agents for acute cystitis in women?
nitrofurantoin, TMP=SMX
fosfomycin has lower efficacy and is more expensive
Outpatient empiric therapy for pyelonephritis?
outpatient-fluoroquinolones
Inpatient empiric therapy for pyelo?
IV
fluoroquinolones
extended spectrum cephalosporins or penicillins
When do you perform imaging for pyelo
If after starting antibiotic therapy, symptoms persist for >72 hours, an US or CT would help look for a perinephric abscess or intrarenal abscess
How long should you treat uncomplicated pyelonephritis for?
How long should you treat complicated pyelonephritis for?
Uncomplicated: 5-7 days
Complicated: 14 days
In pyelo, can you use oral?
Yes in outpatient setting. Inpatient you can switch to oral fluoroquinolones or Bactrim after clinical improvement.
How can recurrent cystitis be prevented?
Daily dose of Bactrim or nitrofurantoin before bedtime, or after sexual activity
Who is at risk for acute bacterial prostatitis?
Diabetes, immunosuppressed, cirrhosis
What are the most common organisms of acute bacterial prostatitis?
E.coli, gram negatives, enterococcus, and in sexually active men N. gonorrhoeae
Treatment of acute bacterial prostatitis
for uncomplicated cases use a fluoroquinolone for 4-6 weeks
What are good oral antibiotics that can be used to step down from IV?
Oral fluoroquinolone, Bactrim (TMP-SMX)
Treatment of choice for pyelonephritis?
Cefoxitin and doxycycline
