Proposed links: lactose, stress, smoking, diet
Patho of IBS
Malabsorption of polyols and fermented CHO
Normal flora process this and as a by product make gas -> flatulence
This disrupts the fx of the GI tract
Molecular signalling defect in serotonin
IBS Mnfts and fx problems occur
What is the function of serotonin
It is involved with secretion, perfusion, motility and pain reflex)
What cells secrete serotonin
Enterochromaffin cells

Varied severity Flatulence Alt. diarrhea, constipation Abdm pain and discomfort Mucoid stools
Dx of IBS
Exclude organic disease
Various scopes (endoscopy, colonoscopy, sigmoidoscopy)
Presentation
Tx of IBS
Avoid offending foods
Decrease stress
PRN Meds: antidiarrheals, antispasmodics, laxatives and SOMETIMES abx
Why do we caution with use of Abx?
It wipes out normal flora so that Mnfts of IBS will decrease but if too much abx are used, then we have a total loss of normal flora and the risk of infection rises a lot
