What is inflammatory Bowel Dz?
What is it characterized by?
Inflammatory changes in the GI tract, caused by an immune response against gut bacteria.
Characterized by lifelong exacerbations
What is Ulcerative Colitis?
UC is characterized by diffuse mucosal inflammation in a continuous pattern limited to the colon. It usually begins in the rectosigmoid area and proceeds proximally, but it may develop in extensive areas of the colon at the same time
What is Crohn’s Dz
CD is characterized by transmural inflammation in a discontinuous pattern that can occur in any part of the GI tract from mouth to anus. The distal ileum and colon are the most common sites of CD involvement
What is the age of onset for IBD?
Bi-modal
More common in 15-30
Second peak is in 60’s
What medications are used for CD?
Steroids for acute flares Sulfasalazine, Mesalamine- best w/CD Immunomodulators: 6-MP/Imuran, Methotrexate Anti-TNF Abx Surgery will not cure
What medications are used for UC?
Mesalamine- maintenance Steroids (prednisone)- for acute flares, steroid dependance is common though Cyclosporine- for steroid refractory pts 6-MP/Imuran- secere, refractory dz Anti-TNF for steroid intolerany Surgery is an option for cure
How are these adz diagnosed and how do they differ?
Diagnosis involves clinical, endoscopic, and histologic criteria
What are the extraintestinal features of uncontrolled dz?
Ocular disorders Apthous stomatitis Ankylosing spondylitis Primary sclerosing cholangitis/hepatobiliary disorders Nephrolithiasis Sacroileaitis Thromboembolic complications Peripheral arthritis Skin disorders
