what are the types of hypothyroidism?
1) primary = thyroid –> 95% of hypothyroidism
2) secondary = pituitary
3) tertiary = hypothalamus
what is the etiology of hypothyroidism?
- autoimmunity
- iatrogenic (radiation and Sx for hyperfx leads to hypofx)
what is Hashimoto’s Thyroiditis?
most common hypothyroidism
describe the patho of hasimoto’s?
1) autoimmunity targeting gland (Ab + T cells) –> inflmtn
2) Antithyroid Abs block TSH binding (prod dt autoimmunity)
3) lymphocyte infiltrates destroy gland (as they did in insulitis)
4) induce atrophy of gland and then fibrosis
what gender is most affected?
90% are middle-aged women
what are the mnftns of hashimotos?
1) hypometabolic state
2) myxedema
3) cold sensitivity
4) fatigued
5) decreased CO
6) decreased CNS fx
7) weak muscle action
8) increased weight
myxedema
mucoid secretion in dermis of skin causing swelling and a waxy appearance
why does cold sensitivity occur?
d/t less heat prod. normally is fixed in body by increased thyroid hormones, but is impaired
why would you feel fatigued?
d/t less ATP prod
why would you have decreased CO?
cardiac muscles do not have required ATP
decreased CNS fx due to?
neurons require ATP for every AP
weak muscle action will cause…
impaired breathing d/t resp muscle weakness
why might you have increased weight?
- not metabolizing nutrients, putting it into storage
- ingest more CHO than metabolized = glucose converted to glycogen and stored in liver
- storage full = glucose stored as lipid elsewhere
