Hypoparathyroidism Flashcards
What is it
Too little PTH is produced from the parathyroid gland.
Causes
Congenital
Acquired
Transient - neonates born prematurely
Inherited (pseudohypoparathyroidism, pseudopseudohypoparathyroidism)
Examples of congenital causes
DiGeorge syndrome
Examples of acquired causes of hypoparathyroidism
Complication of parathyroidectomy or thyroidectomy
Signs and symptoms
Depends on cause
Myalgia
Abdo pain, muscle spasms, seizures, fatigue, headaches, carpopedal spasm, Chvostek’s sign, Trousseau’s sign
Investigations
PTH level Serum calcium Serum phosphate Bloods - FBC, U/Es, LFTs, creatinine, urea ECG - arrhythmias ECHO - cardiac structural defects Radiology
Why is radiology a possible investigation
Pseudohypoparathyroidism patients have shorter 4th and 5th metacarpals
Hypoparathyroidism: Describe levels of: PTH Serum calcium Serum phosphate
PTH - low
Serum calcium - low
Serum phosphate - low
Pseudohypoparathyroidism: Describe levels of: PTH Serum calcium Serum phosphate
PTH - high
Serum calcium - low
Serum phosphate - high
Pseudopseudohypoparathyroidism: Describe levels of: PTH Serum calcium Serum phosphate
All normal
Conservative treatment
High calcium and low phosphate diet
Support for parents
Medical treatment
Calcium and vitamin D supplements
Complications
Renal calculi
Arrhythmias
Cataract formation
Dental problems
