Slowness in initiating and executing movement; may be associated w/ stoppage of ongoing movement

PD
What is seen in static stance?
P LOB
Ddx w/ CVA…
Rigidity seen on both sides of joint (CVA only on 1 side for synergy pattern)
Key tests and signs: movement
Able to move against gravity
Arrests in ongoing movement during functional tasks
Key tests and signs: postural control
Delayed timing of postural adjustments in response to or in preparation for a movement
LOB P
Inability to use appropriate postural control strategy in context
Key tests and signs: STS
Slow or lack of preparatory movement
Assistance w/ initiation
LOB on termination
Unable to shift COM forward
Key tests and signs: gait
Difficulty initiating ambulation
Often requires assistance 2/2 arrests in ongoing movement (i.e., freezing)
Unable to regulate step length
Associated signs: muscle tone
Rigid w/ PROM of UE, LE, or trunk
Associated signs: nonequilibrium coordination
Undershoots movement when aimed toward a target
Slowness or arrests in reciprocal movement
Differential movement dx:
Force production deficit
Cognitive deficit
Expected outcome
When mild, may see improvement in step length and consistency of foot placement
Improvement in use of adaptive strategies in more severe forms
Likely to fall
Associated medical dx:
CVA
PD
Extra-pyramidal syndromes
Dementia
