Statin MOA
inhibition of HMG coa reductase
Statin effect
great for lowering ldl, some lowering of TG and increase in HDL
Statin that doesn’t use Cyp 450 system
pravastatin- less drug interactions and less chance for rhabdo
Cyp3A4 statins
Lovastatin
Simvastatin
Atorvastatin
inhib of 3a4: grapefruit juice, cyclosporine, ritonavir, ittraconalol, erythromycin
induc of 3a4: phyenytoin, barbiturate, rifampin, thiazides
Cyp 2c9 statin
Fluvastatin
inhibit of 2c9: azole drugs
Cyp 2C19 statin
Rosuvastatin
State AE and contra
AE: myalgia, and RHABDO (less with pravastatin)
Contra: pregnancy and liver ailure.
GEMFIBROZIL inhibit OATP2 transport into liver…risk for rhabdo increases
Bile acid binding resins
Chelestyramine, colestipol, colesevelam
Bile acid binding resins MOA
bind to bile acids and prevent respsorption in intestine
Bile acid binding resins effect
modest decrease in LDL (use in pregnant women who cant take statins…or in combo with statins)…but INCREASE IN TGs
Bile acid bindin resins AE and contra/ drug
AE: well tolerated, but can cause lack of absorption of vit (ADEK), …NOT colesvelam
also interacts with any drugs (NOT colesevelam)
contraindications: high triglycerides
Ezetimibe MOA
Cholesterol absorption inhibitor…works by inhibiting Niemann pick C1 like protein (NPC1L1)
Ezetimibe effects
reduce LDL minimally…no other effects…combine w/ statin
Ezetimibe SE
well tolerated, some GI
PCSK9 inhibitors
Alirocumab
Evolocumab
PCSK9 inhibit MOA and indications
antibodies inhibiting protein PCSK9…which destroys LDL receptors
indications: heterozygous FH, r not achieved goals with Statins
Lopmitapide MOA and indication
indication: homozygous FH
moa: inhibitor of MTP (stopping assembly of chylomicrons and VLDL
Mipomersen MOA and indication
indication: homozygous FH
MOA: reduces expression of apoB100, decreasing produciion of VLDL
Niacin effects
decrease LDL, raise HDL (best), and decrease TG. Reduce risk of MI and used in combo withothers
Niacin MOA
dec lipolysis= decreasing FFA and VLDL inhib DGAT2= dec VLDL synthesis inhibit APOCIII= inc VLDL clearance increase apoAI expression: inc HDL prod dec Lp(a)= dec thrombosis
Niacin AE
Skin flushing (can help with NSAID)
increased risk of Gout
Peptic ulcers worst
