Antiflammitory
Suppresses immune system
Accumulates in WBC’s stabilizing lysosomal membranes and inhibiting enzymes such as collagenase that causes cartilage breakdown
Inhibiting toll-like receptors (TLR’s)
Slows progression of RA
Hydroxychloroquine Use
Analgesia reduce joint swelling, pain
Maintenance of joint function
Organ Rejection
RA Pain and Stiffness
Hydroxychloroquine Dosage
Initial: 400-600mg daily in a single dose or 2 equally divided doses
Maintenance: 200mg/daily
Hydroxychloroquine Contraindications
Hypersensitive to Medication
other 4-aminoquinoline compounds
Hydroxychloroquine Interactions
Ampicillin
Antacids
Digoxin
Hepatotoxic or Nephrotoxic drugs
Hydroxychloroquine Education
Take w/ Meals
Take as prescribed
Possible Visual reactions such as halos, blurred vision (need frequent eye exams)
Notify provider in muscle weakness develops
Keep drug out of reach from children
Avoid alcohol
Educate oncompliance and adherence as treatment regiment can be six months or greater
May have increased sensitivity to light-wear sunglasses
Hydroxychloroquine Complications
Anxiety
bleeding gums
blindness
blistering, peeling, loosening of the skin
blood in the urine or stools
blurred vision or other vision changes
