HIV therapy Flashcards
What is the MoA of Maraviroc?
CCR5 antagonist, binds CCR5 on CD4 T cell
Inhibits Entry
What is the MoA of Enfuvirtide?
Entry Inhibitor
Binds gp41 subunit of the viral envelope glycoprotein, preventing fusion
What is the hallmark adverse reaction for enfuvirtide?
Injection site irritation - nodules erythema
What is the MoA of Non-nucleoside reverse transcriptase inhibitors?
Bind directly to the reverse transcrptase
Do not need phosphorylation, not incorporated into viral DNA
What is the major side effect of NNRTIs?
Rash
SJS - highest incidence with Nevirapine
What is the MoA of nucloside reverese transcriptase inhibitors?
Phopsphorylated intracellularly then incorporate themselves to block reverse transcriptase
Similar to building blocks of RNA and DNA
What are the hallmark ADRs of Abacavir?
Hypersensitivty reactions
Need HLA-B5701 screening
What are the hallmark ADRs of didanosine?
Peripheral neuropathy
Pancreatitis
Lactic acidosis with hepatic seatosis
What is the hallmark ADR of emtrictabine?
Hyperpigmentation
What are the hallmark ADRs of stavudine?
Peripheral neuropathy
Lipoatrophy
Lactic acidosis/severe hepatomegaly with hepatic steatosis
What are the hallmark ADRs of zidovudine?
Bone marrow suppression
Lipatrophy and myopathy
Hyperlipidemia, Insulin resistance and DM
What is unique about Tenofovir that separates it from other NRTIs?
Nucleotide - slightly different mechanisms allwoing continued efficacy against HIV strains resistant to other RTIs
Which is the preferred Tenofovir product?
TAF - tenofovir alafenamide
TDF is associated with renal tubulopathy
What is the MoA of integrase inhibitors?
Inhibits strand transfer of viral DNA to host cell DNA
What is the MoA of Protease inhibitors?
Inhibits HIV-1 protease, responsible for cleaving large viral polypeptides into small functional virons
