Infective endocarditis
infection in heart due to bacteria, entry into blood steam for bacterium during treatment, antibiotic prophylaxis taken 1 hour before treatment
types of diabetes:
Pre-diabetes - at risk
type 1 - insulin deficient
type 2 - can’t use insulin
ges. mellitus - glucose intolerance
diabetes mellites risks
Hypoglycaemic episode - have sugar available. slow healing.
Don’t treat uncontrolled unless medical clearance.
- controlled: A1C 7.0%, glucose meter 70 - 130 mg/dl
Uncontrolled: A1C 11.1% ^. GM 200 +
Rheumatic heart disease
Cardiac manifestation of rheumatic fever. prophylactic is not required, may be needed. Use antimicrobial mouth rinse
valvular heart defects
Any cardiac condition for which antibiotic prophylaxis is reccomended
Hypertension
constant elevation of blood pressure
Sys: 160mmHg +
Dis: 100 mmHg +
Coronary heart disease
not enough blood flow from coronary arteries into the heart, if unstable angina postpone , nitroglycerin within reach. if MI in last 30 days postpone
Dysrhythmias and arrythmias
dysfunction of heart rhythm, premedication I unusual is CIED is older than 6 months
congestive heart failure
diminished cardiac output, may be prone to gagging, needs to be upright, limit use of ultrasonic
Cardiovascular surgery
angioplasty (tiny balloon)
coronary bypass: no contradictions
closed heart surgery : no contradictions
heart transplant; big concern, talk to cardio
Cancer
active treatment causes immunosuppression, consult physician, examine lesions every appointment
AIDS, HIV
immunosuppression, consultation for prophylaxis. Maintain standard precautions
Asthma
chronic inflammatory disease characterized by an increased responsiveness of the bronchial airways. stress, semisupine, avoid aerosol, avoid nitrous oxide, bronchodilator
Chronic obstructive pulmonary disease
chronic irreversible obstruction of airflow, semisupine position,
Tuberculosis
active TB should not be treated
