Patient presentation
travel history weight loss, night sweats, cough, fatigue and headaches
crackles on auscultation
increase neutrophil count, increased lymphocytes, increased monocytes
Where is organism commonly acquired?
along Ohio Mississippi valleys; central and South America, India, Africa and southeaster Asia
Where does organism grow?
in soil contaminated with bat, pigeon, bird droppings
who has higher risk of acquiring?
construction workers, agriculture workers, spelunkers, immunocompromised
treatments
Oral intraconazole (>4 weeks pulmonary symptoms)
Severe infections: amphotericin B
Lab Culture demonstrates
dimorphism- organism grows as mold at 25C
in tissues at 37C organism appears as yeast
Lab stain demonstrates
GMS shows yeast seen in macrophages (tissue phase)
Grocott methenamine silver (GMS) stain is commonly used for the identification of fungi on cytosmears and tissue sections.
Lab skin test
DTH Delayed-type hypersensitivity (DTH) skin testing
causes weight loss/cachexia
cytokine response, TNF alpha
reactivation occurs under what conditions
immunosuppressed - HIV, transplant, TNF-alpha inhibitors
pathology
inhaled spores enter lungs in which alveolar macrophages trap them and they change to yeast form. they are transported to lymph node where they can lie latent forming a granuloma. within time calcified fibrinous granulomas with caseous necrosis in the lungs hilarious and mediastinal areas are demonstrated and may also be seen spleen, liver, bone marrow
Lab Xray demonstrates
multiple bodular infiltrates/ enlarged hisar and mediastinal nodes and popcorn calcification
