Which specialty tests are used to test for piriformis syndrome?
Log roll and FAIR test
What is a log roll test?
Internally and externally rotate the thigh
Pain posteriorly = piriformis syndrome
Pain anteriorly/groin region = intra-articular pathology (SCFE, AVN, FAI, etc)
Clicking sound - labral tear
What is a FAIR test?
Flexion, adduction, internal rotation
+ test = pain produced in sciatic/gluteal region
Which tests are used to evaluate for psoas syndrome?
Thomas test and FABER (both positive)
What is a Thomas test?
Pt supine with legs hanging off the table
Pt then pulls knees to chest and extends one leg
+ test = inability to full extend the leg or the other leg raises off the table
What is a FABER test?
Flexion, abduction, external rotation
Groin pain = intra-articular disorders (labral tear, FAI, OA)
Also iliopsoas bursitis/strain
Which tests are used to evaluate for trochanteric bursitis?
Ober test and jump sign
What is the jump sign?
Pressure is applied to the greater trochanter with the pt seated
+ test = pt withdraws or jumps with pressure
What is the Ober test?
Gluteus medius: pt is in the lateral recumbent position with top hip extended and knee flexed at 45-90 degrees and physician is adducting the hip
+ test = pain at greater trochanter or inability to adduct
Which tests evaluate for intra-articular pathologies?
Log roll, FABER, scour and labral load/distraction
What is the C sign?
When asked to point to the pain the pt will make a C with their hand over their hip
What is labral loading and distraction?
Labral loading: flex pt’s knee and hip to 90 degrees, load into femur towards innominate; + = pain
Distraction: distract femur away from innominate; + = improvement of pain
What is the scour test?
Flex and externally rotate the hip
Load into socket and articulate through annular ROM
+ = pain
What is the straight leg raise?
With pt supine flex the hip with the knee fully extended
Resisted SLR = SCFE or femoral acetabular impingement (FAI)
