Signs and symptoms of hepatitis
Many asymptomatic. Malaise, nausea, body aches, headaches, loss of appetite, low grade fever, abdominal pain
Etiology of Hepatitis
Infection, immune related: autoimmune/PBC/PSC. Chemical/toxin, meds, ischemia. Hereditary: Wilson’s, hemochromatosis, alpha 1 anti trypsin. Pregnancy, alcohol, non alcoholic fatty liver disease
auto immune hepatitis
more common in females. ALT/AST increased, ANA positive, anti smooth muscle antibody and anti LKM antibodies positive. Portal triaditis. treated with steroids or immunemodulators ie: Azathioprine
Primary Biliary Cirrhosis
Immune system selectively attacks bile ducts. >80% of pts have an anti mitochondrial antiody. Elevated cholesterol common. High risk of osteoporosis. Treat with Urso (bile acid supplement)
Primary sclerosing cholangitis
Immune mediated stricturing of the biliary tree. Involves intra and extra hepatic duct systems. Associated with ulcerative colitis and crohn’s disease. May be ANCA+. No medical therapy available, even with transplant
Causes of toxin induced hepatitis
Tylenol, amanita phalloides, white phosphorus, carbon tetrachloride
