- active major bleeding
- recent history of ulcer disease
- advanced renal disease

- observe injection site for hematomas, ecchymosis, inflammation

Implications
A
- double check med - HIGH ALERT
- assess for signs of bleeding or hemorrhage: bleeding of gums, nosebleeds, unusual bruising, black tarry stools, hematuria, all fall w/in lab values listed below
- give into subcut tissue
- alternate injection site daily btw L&R anti/postolateral abdominal wall & upper thigh
- inject full length of needle into tissue at 45/90º angle into skin fold held btw thumb & forefinger
- lab look out: Platelets (150-400,00), PTT (20-45 sec), INR (1-3 sec)
