Heme drugs Flashcards
Heparin- Indications
Prevention of DVTs, treatment of PEs, pregnancy
Heparin- MOA
Binds to antithrombin III, inactivates coagulation factors
Heparin- AE
hemorrhage, thrombocytopenia, hypersensativity
Heparin- Monitoring
Bleeding time
LMWH- Indications
Prophylaxis of DVT, MI,
LMWH- MOA
Selectively accelerates interaction of antithrombin with factor Xa
LMWH- AE
Fewer than heparin, less frequent bleeding,
Fondaparinux- Indications
Prophylaxis of DVT, acute PE & DVT in combination with warfarin.
Fondaparinux- MOA
Selectivly inhibits factor Xa by selectivly binding to antithrombin III
Fondaparinux- AE
Bleeding, no throbocytopenia
Warfarin- Indications
Prevention of progression or recurrence of DVT or PE following initial heparin tx. Prophylaxis of MI, a-fib
Warfarin- MOA
Antoagonize cofactor function of Vit. K
Warfarin- AE
Black box for bleeding, pregnancy, drug-drug interactions
Warfarin- Monitoring
INR regularly to adjust dose
Dabigatran- Indications
Prevention of stroke, embolism, a-fib. Does not require INR
Dabigatran- MOA
Direct thrombin inhibitor
Dabigatran- AE
Bleeding, GI, fewer drug-drug interactions than warfarin
Rivaroxaban- Indications
Prophylaxis of DVT, stroke, PE
Rivaroxaban- MOA
Selective inhibition of factor Xa
Rivaroxaban- AE
Risk of thrombosis with discontinuation, Bleeding, epidural/ spinal hematoma, back pain
Apixaban- Indications
Prophylaxis of DVT, stroke,
Apixaban- MOA
Selectively inhibits factor Xa
Apixaban- AE
Risk of thrombosis with discontinuation, Bleeding, epidural/ spinal hematoma, anemia
Ticlopidine- Indications
Prevention of TIA, strokes for patient with previous events, with aspirin to prevent thrombus
