Iron Metabolism and Interpretation of Iron Studies + Ferritin

A

Iron –>stomach acid –> enterocyte (saturatable) –> transferring –>cells or liver (stored in ferritin).

RDW - not useful
Free erythrocyte protoporphyrin - not useful
serum iron - not useful
TIBC + Transferrin sat: if high TIBC low transferring sat, likely iron def. If serum iron high, TIBC low, transferring sat high, likely hemochromatosis, otherwise not useful.

Ferritin - < 15 ng/mL = Fe anemia. >100 ng/mL rules out iron def. Otherwise should be higher than patient’s age. Can have fatigue/alopecia/restless legs with ferritin <50/100 in absence of anemia.
