heart failure meds Flashcards
Beta blockers used in HF
metoprolol succinate
bisoprolol
carvedilol
Metoprolol dosing
- 25-12.5 mg/day
goal: 200mg/day
Bisoprolol dosing
1.25mg/day
goal 10 mg/day
Carvedilol dosing
3.125mg BID
goal is 25mg BID
Lisinopril dosing
start w/5 mg
goal is 20mg
can dbl the dose every other week, if starting w/b-blocker have to alternate the titration, including starting (every other week will be up titrating either the b-blocker or the ACE/ARB)
aldosterone blockers
spironolactone
eplirinone
spironolactone dosing
12.5 - 25mg/day
eplerenone dosing
25 mg/day
vasodilators
hydralazine
isosorbide dinitrate
loop dirutics
furosemide
bumetanide
torsemide
furosemide dosing
10-40mg/day
bumetanide dosing
.5-1 mg/day
torsemide dosing
10-20 mg/day
digoxin dosing
.125 mg/day
every other day in elderly
nitroprusside
vasodilator acting directly on vascular smooth muscle,
nitroglycerine
venous dilator, acts as nitric acid donor
nesiritide
recombinant BNP
reduces sympathetic stimulation
inhibits renin-angiotensin-aldosterone system
results in vascular smoother muscle relaxation, balanced vasodilator and diuresis
Dopamine
inotropic & vasopressor activities
converted to NE, activates alpha, beta, dopaminergic receptors
Dobutamine
b-agonist
bind to beta 1 receptor and increases calcium influx during systole
Milrinone
Phosphodiesterase inhibitor
increases intracellular cAMP whicn increases intracellular calcium
