Heart failure drugs Flashcards
Furosemide MOA and use
MOA: powerful diuretic that acts on the thick ascending limb. Reduces pre-load and edema. Vasodilating effect on pulmonary vessels.
Use: HF, pulmonary edema
Furosemide AE
-ototoxicity
-hypovolemia
-hypokalemia
When are diuretics recommended for patients with heart failure
recommended for all HF patients with fluid retention
dont use alone: in combo with ACEI or Bb
When is Lisinopril (ACEI) contraindicated?
in pregnancy
function of ACEI in heart failure
improves mortality rate and reduced disease progression
Lisinopril MOA and use
MOA: blocks angiotension converting enzyme. reduces vascular tone and aldosterone activity
Use: HF, HTN, diabetes
Lisinopril AE
cough
renal damage
hyperkalemia
contraindicated in pregnancy
When are beta blockers indicated for a heart failure patient, when is it contraindicated ?
for long term management of stable chronic HF
-contraindicated in acute patients
Carvedilol MOA and use
MOA: A+B blocker. poorly understood
use: CHF
Carvedilol AE
cardiac depression
metoprolol MOA and use
MOA: B blocker , poorly understood
use: CHF
metoprolol AE
cardiac depression
Spinolactone
MOA
use
AE
MOA: aldosterone antagonist in the kidneys
use: CHF,aldosteronism
AE: hyperkalemia and gynecomastia
what should digitalis be used in conjuction with for HF
ACEI or beta blocker to lower mortality rates
Digoxin MOA,use, AE
MOA: inhibits NA/K+ pump
increases intracellular na , decreases calcium expulsion and increases cardiac contractility
use: HF with rapid atrial fibrilation, nodal arrthymias
AE: arrthymogenic, nausea, vomiting, diarrhea, visual changes and endocrine changes (Rare)
