Define a nutritional assessment
Systematic process of collecting and interpreting information in order to make decisions about the nature and cause of nutrition related health issue that affect an individual
Define nutritional screen
Brief risk assessment which can be carried out by any healthcare professional and which may lead to a nutritional assessment by a dietician
e.g. MUST
ABCDE
A = anthropometery 9weight, BMI, skin fold, weight change) B = biochemistry (FBC, folate, b12 etc.) C = clinical disease states or symtpoms Dietary = energy and fluid requirements, diet assessment E = environment factors (social and physiological)
Dietary History Questions
Food diary, what is their typical food and fluid intake?
Are they eating 3 meals a day?
Are they snacking?
Are they having regular fluids, 6-8 glasses a day?
Are they having carbs, what carbs and proteins?
Portion size? (at least size of fist and amount to 1/3 of each plate)
Are they eating at least 1 fruit/veg portion with each meal a day?
Do they have access to essentials daily?
Are they taking nutritional supplements?
What does the eatwell guide comprise of?
1/3 vegetables and fruits
less than 1/3 carbs
small amounts of dairy/beans/pulses
