Elevated ICP therapies/meds
Raise head of bed
Hyperosmolar therapy
Hyperventilation to PCO2 of 35 (if actively herniating only )
Hyperosmolar therapy meds
Mannitol
3% HTS
Mannitol dose (elevated ICP)
1g/kg q6hr
- watch for HOTN 2/2 diuresis
3% HTS dose (elevated ICP)
Adults:
- 250mL over 10 min then 50-100mL/hr
Peds: rule of 3’s (3mL/kg of 3% over 30 min)
- 2-5mL/kg over 30 min
SAH/ICH BP goals?
SBP <140mmHg (institution dependent protocol)
SAH/ICH medications
Nicardipine
Labetalol
Nimodipine
Clevidipine
Nicardipine dose (SAH/ICH)
5-15mg/hr
Labetalol dose (SAH/ICH)
10-20mg IV bolus q 3-5 min
Then
0.5-2mg/min
Nimodipine dose (SAH/ICH)
60mg PO q4h (non-traumatic SAH only, started w/in 4 -21 days)
- likely not started in the ED
- can cause HOTN
Clevidipine dose (SAH/ICH)
Start 1-2mg/hr
