Hx presentation: 30-50 y/o, women, smokers, FH of RA
Sx presentation: Pain and stiffness in multiple joints (especially wrists, PCP, and MCP), morning stiffness lasting more than 1 hour, fatigue, weight loss, anemia
Exam: Boggy swelling, synovial thickening in joint exam
Criteria
Have at least 1 joint with definite clinical synovitis
Synovitis not better explained by another disease
Low-high positive Rheumatoid factor or low-high positive ACPA
Abnormal CRP or abnormal ESR
>6 weeks
What should we check for in all children with arthritis?
Idiopathic inflammatory eye disease (Uveitis), should be screened by ophthalmologist in all children with arthritis
How does HIV often present?
Adults: Mono-like or flu like. Sore throat, malaise, fever.
Kids: SCID like, failure to thrive, diarrhea.
What are the treatments for psoriasis?
Mild (<5% body surface area)- Topical corticosteroids, Vitamin D, Tazorac
Moderate (~5% body surface area)- Systemic biologic therapiesn
Severe (>5% BSA) - TNF inhibitors for psoriatic arthritis
