- return blood product & IV tubing to blood bank for further testing
- notify blood bank when a/e occur
- obtain blood and urine sample ASAP to help identify the type of blood transfusion reaction
- documentation
- keep IV line open w 0.9% saline (use a new IV set) for emergency medication if required
