  1. stop transfusion immediatly
  2. inform dt
  3. monitor vital signs q15min until stable
  4. return blood product & IV tubing to blood bank for further testing
  5. notify blood bank when a/e occur
  6. obtain blood and urine sample ASAP to help identify the type of blood transfusion reaction
  7. documentation
  8. keep IV line open w 0.9% saline (use a new IV set) for emergency medication if required
