presents with swelling of joint with acute pain, inflammation, warmth

Allopurinol
A
Needs renal dosing
max 800mg day
100mg d, titrate up 100mg/d every 2-5 weeks till uric acid is <6
Average dose is 300mg/d

Allopurinol and Stage 4 kidney disease
A
Stage 4 kidney disease: start with 50mg then titrate to therapeutic dose

Febuxostat
A
40mgd, MAX 80mg if UA is not below 6 in 2 weeks
Do not use with asian pt who test positive for the HLA-B-5801 allele

Considerations of Gout therapy
A
urate levels begin to drop as soon as 2 weeks
draw labs with each titration
May take up to 6mos to reach therapeutic levels
Monitor liver function
