What causes Goodpasture’s disease?
A
Autoantibodies to type 4 collagen, an essential component to the glomerular basement membrane

Where is type 4 collagen also found?
A
Also found in the lung, pulmonary haemorrhage can occur. esp. in smokers

How do patients present?
A
- Haematuria / nephritic syndrome
- AKI may occur within days of onset of symptoms

What is the treatment?
A
- Plasma exchange
- Corticosteroids
- +/- cytotoxics

What is important regarding the timing of treatment?
A
If it is started early, full recovery is possible with relapses being rare.
Renal prognosis is poor if on haemodyalisis at presentation.
