Grade I
Incipient/ early-stage lesion
Primarily affects soft tissue
Suprabony pockets (slight bone loss)
No radiographic evidence of bone loss
Grade II
No through and through interradicular bone loss
Cul-de-sac with definite horizontal component
Distinct horizontal destruction of furcation
Portion of PDL remains intact
Partial penetration of Naber’s probe
Possible radiographic evidence
Grade III
Through and through destruction of bone and soft tissue
Bone is not attached to the dome of furcation
Furcation is filled with soft tissue hence not clinically visible
Pocket formation
Able to pass Naber’s probe through
Radiographic evidence (small triangular radiolucency)
Grade IV
Interdental bone is destroyed
Furcation is clinically visible due to gingival recession
Tunnel exists between roots of affected teeth
Radiographic evidence is noted
Periodontal probe passes from one aspect to another
(through and through tunnel)
