Gladding Ch. 7 Flashcards
4 Common Curative Elements in Psychotherapy and Percentage of improvement they account for (Lambert)
- extratherapeutic (40%)
- therapeutic relationship (30%)
- expectancy, hope, placebo (15%)
- Model & technique (15%)
2 primary components of family therapy
content and process
content
details and facts. a good rule to follow in dealing w/ facts is to ask “how” questions alongside what, where, and when questions
process
how information is dealt with in an interaction
Battle For Structure
struggle to establish parameters under which therapy is conducted. a key component is for the therapist to delineate the donditions under which treatment will occur. It is the therapist’s reponsibility to establish structure
SOLER
S - face client/family squarely O- adopt open posture L- lean forward in direction of family E- eye contact R - relaxation
Battle for Initiative
must be won by family.
Pre-session tasks
therapy begins moment of first interaction. make hypotheses about family dynamics when evaluating intake info. form preliminary diagnosos of what is happening w/in the family.
Ask:
“what happened”
“why did it happen”
“what can be done about it and how”
joining
establishing trust
frame
bateson. a perception or opinion that organizes one’s interactions so that “at any given time certain events are more likely to occur and certain interpretations of what is going on are more likely to be made”
family dance
families have personalities. they come into therapy and display these personalities in both a verbal and nonverbal manner.
triangulation
scapegoating a third member to relieve tension between 2 members
enmeshment
(overinvolvement physically and/or psychologically)
distancing
(isolated separateness, physically and/or psychologically)
reframing
art of attributing different meaning to behavior so the behavior will be seen differently by the family
middle phase of treatment
family pushed to make changes and breakthroughs. involve peripheral family. circular questioning. seek to connect family members. emphasize change. establish contracts and quid pro quo relations
termination
4 steps
- orientation
- summarization
- discussion of long-term goals
- follow-up and relapse prevention
3-session termination process
setting date
next-to-last session
farewell session
Process of Family Therapy
- Initial Sessions
- Middle Phase
- Reorientation and CHange
- Termination
Common Factors Model
○ Extratherapeutic Factors (40%) - anything in
the client environment that leads to positive
change
○ Therapeutic Relationship (30%) - rapport and
the therapist’s willingness to display empathy,
warmth, concern, and adopt the language of
the family
○ Expectancy, Hope, and Placebo (15%) -
when the client expects therapy to be helpful, it
typically is.
○ Model and Technique (15%) - when the
therapist is comfortable and utilizes their
approach effectively
content
Content: the “what” of the family (details and
facts)
process
Process: the “how” of the family (how is
communication and energy exchanged)
flights into health
Early / Easy Resolutions: be careful of a family’s
“flights into health” - i.e. the euphoria and/or relief
that comes with talking (which is not behavior
change)
battle for structure
Battle for Structure: The therapist is in
charge of the session.
SOLER positioning
stance, openness,
leaning in, eye contact, and relaxed
posture.
Battle for Initiative
Battle for Initiative: the family must be
motivated to make changes. If no
progress is seen or made, the family will
likely either drop out or simply “go
through the motions.”
ii. Invite clients to imagine futures where
they get what they want, and link those
outcomes to small, concrete steps.
attending to non-verbals
Attending to Nonverbals: where do the
families eyes, bodies, and hands go? What is
being said, and what is being shown?
general process
clarify communication among family members, overcome resistance, rectify dysfunctional behaviors
Lambert - 4 curative elements in psychotherapy and the percention of improvement they account for
- extratherapeutic 40%
- therapeutic 30%
- expectancy, hope, placebo 15%
- model and technique 15%
redirection
a method by which the therapist asks the couple or family to attend to the process of their relationship instead of its content
process of therapy
first stage - beginning sessions. clarify concerns and difficulties.
second stage - working sessions. help make changes that will work and make relationships better.
final stage - termination. complete process of change and end sessions
pre-session task
preliminary diagnosis of what is happening w/in the family
pre-session questions
what happened?
why did it happen?
what can be done about it and how?
4 steps of termination
- orientation
- summarization
- discussion of long-term goals
- follow-up and relapse prevention
initial sessiosn
observe power, coalition, roles, rules, patterns of communication.
ask about a typical day in life of family
middle sessions
help fams become aware of behaviors, reorient
