7
Q

E histolytica Dx

A

2 methods: 1)cysts with multiple nuclei in stool (but cannot differentiate between this an Entamoeba dispar.) or 2)serology with trophozoites containing RBCs in cytoplasm

17

A

Giardia

18
Q

Cysts on micro

A

Giardia, E histolytica

19
Q

RBCs in cytoplasm/multiple nuclei on micro

A

E histolytica

20
Q

Bloody diarrhea

A

E histolytica

21
Q

nonbloody diarrhea

A

Giardia, Cyclospora cayetanesis

22
Q

Can nonbloody diarrhea from Giardai/Cyclospora happen in immunocompetent people?

A

Yes

23
Q

E histolytica mvmt?

A

Pseudophilia (creeping cytoplasm projections)

24
Q

E histolytica epidemiology?

A

Developing world.

25
Q

Problem with E histolytica stool sample analysis?

A

cannot differentiate between E histolytica and E dispar.

26
Q

Most common E histolytica Sx?

A

none, usually ASx

27
Q

E histolytica trophozoite-> cyst progression

A

trophozoite->precyst (2 nuclei+chromatoid bodies/ribos+food vacuoles)->cyst (4 nuclei)

28
Q

E histo Active vs carrier?

A

Stool sample-active has RBCs in cyst cyto, carrier does not

29
Q

Metrodinazole train (indicated bugs, AEs)

A

The metro runs over Ehistolytica, Giardia, Trichomonas, and anaerobic cocci/bacilli (GET out of his way). No drinking on train (upset stomach, disulfaram effect). If you eat the train get metallic taste in mouth.

30
Q

Giardia outbreaks occur when…

A

sewage contaminates drinking water

31
Q

Giardia MOI

A

ingestion->cyst->intestinal coating, which prevents fat absorption. Does not invade wall so no blood

32
Q

Crypto prevalence

A

25% of US has been infected

33
Q

Crypto clinical scenarios

A

Contaminated water sources, infants in daycare, travelers

34
Q

Severe diarrhea in immunocompromised person

A

Cyclospora, Isospora (also think about crypto)

35
Q

Cyclospora/Isospora Dx

A

stool microscopy (ACID FAST)

36
Q

Acid fast GI Parasites

A

Cyclospora/Isospora, Crypto

37
Q

Cyclospora/Isospora Tx

A

trimethoprem-sulfamethodaxole

38
Q

Cyclospora associated food

A

raspberries
