What is GERD
A
Occurs when stomach acid frequently flows back into the esophagus and this acid reflux can irritate the lining of the esophagus

What is the cause of GERD
A
Weak and incompetent lower esophageal sphincter (LES)

A
30-60mins after meals; evening onset

What may retard esophageal acid clearance and contribute to GERD?
A
big hiatal hernia

Is there radiating pain?
A
Yes, radiates to the throat, shoulder or back

What reduce the LES tone?
A
- caffeine
- fats
- chocolates
- alcohol
- smoking

Symptoms of GERD
A
- heartburn
- regurg
- hypersalivation
- difficulty swallowing
- dyspepsia

Therapeutic managements for GERD
A
- antacids (eg. calcium carbonate)
- antacids + alginic acid
- H2 blocking agents / proton pump inhibitors
- surgery

What meds to avoid?
A
Anticholinergics (eg.antipsychotics, benztropine, cogentine)

Nur management for PRE-endoscopy
A
- 6-8hrs fast
- remove dentures
- administer sedatives

Nur management for POST-endoscopy
A
- if pain or bleeding: inform staff nurse
- withhold foods, fluids & oral meds until patient is fully alert
- vital signs

Nur management for GERD
A
- small frequent feedings
- elevate head position when sleeping
- weight reduction if indicated
- avoid eating 2 hrs before bedtime
- avoid tight-fitting clothes
