What is the Care Programme/Plan Approach?
A
A model for psychiatric care in the community with the aim to improve and standardise the planning of care for psychiatric patients.
- Detailed Assessment (Medical, Social, Risk).
- Assigning Key Worker (Co-ordination, Liaison, Planning).
- Care Plan - records identified needs of the patient and how they will be sorted out with the patient present.
- Progress monitored by Key Worker who reports back to CMHT.
- Formal CPA Meetings.

- Direct assistance in symptom management.
- Follow-Up in assertive manner.
- Replace total support of the hospital with comprehensive, intensive and flexible support in the community.
