What is the Care Programme/Plan Approach?

A

A model for psychiatric care in the community with the aim to improve and standardise the planning of care for psychiatric patients.

  1. Detailed Assessment (Medical, Social, Risk).
  2. Assigning Key Worker (Co-ordination, Liaison, Planning).
  3. Care Plan - records identified needs of the patient and how they will be sorted out with the patient present.
  4. Progress monitored by Key Worker who reports back to CMHT.
  5. Formal CPA Meetings.
