Obstetric risk factors for GBS (early onset)
A
Previous infant with EOGBS GBS bacteruria this pregnancy Spontaneous onset of labour < 37 ROM > 18 hours Intrapartum fever >38 Known carriage of GBS in current pregnancy Chorioamnionitis clincally Other twin with current EOGBS

Universal screening (35-37)

LVS + anorectal swabs
Selective enrichment broth medium
If negative, still need IV Abx in labour if has had previous infant with GBS or GBS bacteruria in this pregnancy

Aus intrapartum chemoprophylaxis with no signs of sepsis
A
Benpen 3g IV load, then 1.8g. Q4H
If pen allergic, clindamycin 600mg IV Q8H OR vbac 1g IV Q12h

amox 2 g IV Q6H, gent 4-6mg/kg, metro 500mg IV Q12H
