white spots on nails

- iron deficiency e.g. due to GI bleed

- IBD, cirrhosis, lymphoma, coeliacs disease
tendon xanthomata, corneal arcus and xanthelasma
- hyperlipidaemia e.g. due to PBC, cholestasis
Dupuytren’s contracture
- CLD, DM, heavy labour, trauma, phenytoin, familial
palmar erythema
- CLD, pregnancy, hyperthyroidism, RA
pale palmar creases and conjunctival pallor
- anaemia
Flapping/Hepatic Tremor
- hepatic encephalopathy, respiratory/ renal failure
Bruising
- CLD due to thrombocytopenia, clotting factors, falls
IVDU marks
- risk of hepatitis B&C
hep A more travel related
Cushingoid
(moon face, acne, hirsute)
- alcohol excess
angular stomatitis and glossitis
- iron/folate/B12 deficiency
Fetor hepatitis
(musty, sweet odour of the breath)
- hepatic failure
mouth ulcers, episcleritis and conjuctivitus
- associated with IBD
gynaecomastia
- CLD, testicular failure, drugs
loss of secondary hair
- CLD
spider naevir
- > 5 suggests CLD
bruising on abdomen due to pancreatitis
CULLEN’S - central/front
GREY-TURNERS - flank
signs of peritonitis
rebound tenderness
guarding
shifting dullness
> 1/5L of ascites
erythema nodosum and pyoderma gangrenosum
- IBD
right hypochondriac pain DDx
gallstones hepatitis ascending cholangitis cardiac and lung causes peptic ulcer disease
epigastric pain DDx
MI, pneumonia
oesophagitis
peptic ulcer disease

left hypochondriac pain DDx
