7
Q

What is the Murphy’s sign

A

Whilst applying pressure in the RUQ, ask the patient to inspire. Murphy’s sign is positive when there is a halt in inspiration due to pain, indicating an inflamed gallbladder

9
Q

Abdo USS features for cholecystits

A
  1. The presence of gallstones or sludge
  2. Gallbladder wall thickness (if thick walled, then inflammation is likely)
  3. Bile duct dilatation (indicates a possible stone in the distal bile ducts)

Q

Medical term for gallbladder stones

A

cholelithiasis

21
Q

Define Gallbladder mucocoele

A

a condition where the gallbladder becomes distended and filled with sterile mucus due to obstruction of the cystic duct.

22
Q

Gallbladder mucocoele Sx

A
  • RUQ pain - dull, persistent, worse post meal
  • N + V
  • Fever
23
Q

Gallbladder mucocoele clinical signs

A
  • Palpable RUQ mass - Distended, non-tender gallbladder
  • Septic - hypotension, Tachycardic, Tachypnoeic
  • Fever
24
Q

Gallbladder mucocoele USS features

A
  • Distended, thin-walled gallbladder with anechoic (no sound) content.
  • No gallstones (unlike typical cholecystitis)
25
Q

Definitice Tx for gallbladder mucocoele

A

Cholecystectomy

26
Q

untreated Gallbladder mucocoele complications

A
  • Gallbladder Empyema – pus accumulation
  • Gangrene and Perforation – Ischemia may lead to necrosis and rupture.
  • Peritonitis – Due to gallbladder perforation.
  • Biliary Peritonitis – Leakage of bile into the peritoneal cavity
27
Q

Medical term for CBD stones

A

Choledocholithiasis

28
Q

Laparoscopic cholestectomy complications

A
  • Bleeding
  • damage to bile ducts
  • damage to bowel, blood vessels and organs
  • VTE
  • Stones left in bile duct
  • Anaesthetic risks
  • Post-cholestectomy syndrome
29
Q

Post-cholestectomy syndrome Sx

A
  • diarrhoea
  • indigestion
  • RUQ pain
  • Nausea
  • Flactulence
  • intolerance of fatty food
