What is coeliac disease?
A chronic immune-mediated systemic disorder in genetically predisposed people, triggered by exposure to dietary gluten leading to small bowel damage and malabsorption

Anti-TTG IgA (min. 6/52 of gluten diet before test)
Gold standard diagnostic for Coealiac
OGD and duodenal/jejunal Bx
Coeliac GI Sx
Abdominal pain
Distension
Nausea and vomiting
Diarrhoea
Steatorrhoea
Coeliac clinical features
- Pallor (secondary to anaemia)
- Vitamin def. from malabsorption (bruising from vit K def.)
- Abdo distention (bloating)
Coeliac derm manifestation
Dermatitis herpetiformis (pruritic papulovesicular lesions over the buttocks and extensor surfaces of the arms, legs, and trunk)
Coealic exclusion lab tests
- Stool MC&S: to exclude infectious cause
- Faecal calprotectin: to exclude IBD
Coeliac histological findings
- sub-total villous atrophy
- crypt hyperplasia
- intra-epithelial lymphocytes
Coeliac Mx
- lifelong adherance to a gluten-free diet
- Annual monitoring for gluten adhearance
- Annual flu vaccine
- Pneumococcal vaccine
