What do aspergillus fumigatus usually infect?
Lungs… hypersensitivity forms fungal balls with eosinophils (Allergic BronchoPulmonary Aspergillosis…ABPA)
What can be used to treat ABPA?
Voriconazole (antifungal)
Prednisone
How do azoles (voriconazole or itraconazole) work?
Binds fungal P450 enzyme (Erg11) –> inhibits synthesis of ergosterol –> build up of toxic lanosterol
What toxicities do azoles have?
Drug-drug interactions
Hepatotoxicity*
Neurotoxicity
Alters hormone synthesis –> avoid during pregnancy
What causes the resistance to azoles?
Altered P450
Upregulation of efflux transporters
