1st line therapy for gout attack
NSAIDs
2nd line therapy for gout attack
colchicine
give <36 hours after attack
colchicine dose
2ts at onset, 1 tab 1 hour later then 0.6mg PO QD-BID 12 hours later
most common side effect of colchicine
diarrhea
complete resolution of gout symptoms w/ colchicine in
24-48 hours
many w/ relief in a few hours
last line therapy for gout attack
corticosteroids & opiods
consider gout prophylaxis if
the pt had severe attack, nephrolithiasis w/ attack, serum urate >10, 24 hr urinary excretion of urate >1000mg, tophi present 2+attacks/yr, renal dysfunction

1st line gout prophylaxis
colchine or NSAIDs
colchicine prophylaxis dose
0.6mg PO QD-BID
CI if CrCl<10
duration of anti-inflammatories for gout prophylaxis
at least 6 months
drug of choice for overproducers
xanthine oxidase inhibitors- allopurinol or febuxostat
allopurinol dose
100mg PO QD
