Distance vision at 10 ft with Snellen chart
Near Vision with Rosenbaum chart

Extra-ocular Muscle Function (6 point cardinal positions) “EOM in tact, no nystagmus)
Eye structure: Eyebrows symmetric, lid smooth, sclera white, conjunctiva pink, cornia clear, no lesions or drainage
Test pupillary light reflex with pen light both eyes testing for individual constriction and bilateral/simultaneous constriction
Test accommodation with focus
State PERLA “Pupils Equal Round Reactive to Light and Accommodating”
Head Section: Ears
Palpate w/ gloves the Pinna, Targus, and Mastoid process for symmetry and no pain
Test hearing w/ whispered voice test bilaterally 1-2 feet from each ear
Head Section: Nose/Sinuses
Palpate w/ gloves nose (mid-line, symmetric, no pain), bilateral frontal and maxillary sinuses (non-tender
Verifies patency of nares by occluding one nostral at a time and sniffing
Inspects internal nares w/ penlight to sensure midline non deviated no redness or polyps
Head Section: Mouth and throat
Inspect structures of the mouth w/ penlight checking lips (pink, moist), gums (pink), teeth (intact and clean), tongue (pink, moist, and smooth), and mucous membrane (pink and moist)
Inspect Throat w/ penlight for hard and soft palate (intact, oropharynx (pink), uvula (midline and rises with phonation), and tonsils either present or absent with or without redness
Neck
Palpates bilateral cervical lymphodes for lymphadenopathy or round, mobile, and non-tender. Trachea is midline
Thorax and lungs
Identifies Rate, Rythm, and quality (count 30 sec X 2, regular and even, unlabored)
Inspect anterior and posterior chest for symmetry and transverse ratio (1:2)
Auscultate anterior chest (5 positions) posterior chest (7 positions) Lateral chest (2 positions)
Identify chest sounds (clear)
Identify presence or absences of adventitious breath sounds (non present)
Cardiovascular/Peripheral Vascular
Have PT lie down with pillow under their knees:
Inspect bilateral JVD
Inspect precordium (no lifts or pulsations)
Identify hr rate Count 30 sec X2 and rythm regular/irregular or radial pulse
Palpate bilateral peripheral pulses:
Carotid
Brachial
Radial
Femoral
Popliteal
Posterior tibial
Dorsalis pedis
Grade 2+=normal, equal and bilateral
Auscultate Anterior chest
(Aortic area) RSB 2nd ICS (S2>S1)
(Pulmonic area) LSB 2nd ICS (S2>S1)
(Erbs point) 3rd ICS (S2=S1)
(Tricuspid area) 4th ICS (S1>S2)
(Mitral area) 5th ICS (S1>S2)
Auscultate Apical pulse (mitral area) Identify BPM and rythm for 1 min
Auscultate bilateral carotid arteries (use bell of stethoscope) one side at a time while holding breath “no Bruit”
