Filling Out the 28e Flashcards
A mom is filling out the 28e for her 3-year-old daughter (her dad died). She comes across section 1 it asks for “relationship to the deceased”. What is she supposed to put down in that area?
DAUGHTER!!! not wife!!!!
A mom is filling out 28e for her 3-year-old daughter (who is in our system and has a Thrivent id of 500100600)
DO NOT write down the daughter’s Thrivent id, it’s asking for the dead dad’s Thrivent id
In the MCC, I wasn’t able to disclose/provide full contract numbers, just the last 3 digits of a contract number
BUT in the death claims area, you are allowed to give the beneficiary a full contract number if he needs it to complete the 28e
The caller has questions about the Guaranteed Lifetime Withdrawal Benefit, he wants to know more about it. How would you help him?
Do not try to figure this out for yourself, it’s impossible! Call the examiner for an answer to his question
A beneficiary, when filling out his 28e, can use his proceeds to directly pay off a…
a loan, an internal loan to be specific (a Thrivent loan) this would be done in section 3 page #3
When it comes to section 3, choosing FPDA, what is the payment date?
It is the date on which Thrivent will process the FPDA, NOT the day on which the bene will receive his first payout. It will be issued the next business day
If the bene is choosing “continue payments” or “take payments more rapidly” he will need the form…
9386C and 9386 respectively
The claimant is synonymous with…
beneficiary. Beneficiary and claimant are one and the same
the beneficiary wants to transfer their (deceased) spouse’s annuity funds into his/her own contract.
Fill out “Transfer of qualified funds” area in section 3
When would the beneficiary need to complete section #11?
NEVER! Section 11 is for the FP
Which sections need to be completed always, no matter what the beneficiary elects?
Section 1, section 2, and section 9 (signature)
If the claim kit doesn’t mention anything about the RMD…
Then you can rest assured the beneficiary does not need to satisfy an RMD for the deceased
Section #8 of the 28e is for…
Establishing your beneficiary. Don’t wanna leave that money in limbo, now do we?
Section 6 is for…
Witholding taxes
Section 5 is for…
Required Minimum Distribution
In what situations would it be necessary for the beneficiary to complete section #10?
If the beneficiary elected DISO or FPDA
“Transfer of Qualified Funds”
Is this a new product? √ yes (oh no! beneficiary doesn’t have a contract number. What do we do?)
No contract #? No problem! Just write “new application” where it asks for the contract number
Section 6 is only ever necessary to fill out if…
If the death benefit is taxable. For a life insurance policy (which is almost never taxable) section 6 wouldn’t need to be completed
Form 9368 is a…
its an application for a new settlement option
Form 9368c is needed if that death benefit money is going to….
going to fund another (existing) Thrivent product
If a trustee or a mom or a dad is filling out the 28e, what’s important to remember in section #9?
Important to, after applying signature, note in what capacity you are signing the document (i.e. “legal gaurdian” or “trustee”)
