chronic non-inflammatory, non-autoimmune diffuse central pain-processing syndrome
-not a primary disorder of muscle or connective tissue
-patients with FMS are more sensitive to other sensory modalities in addition to rpessure
what are the cardinal manifestation
widespread pain
tenderness on exam
fatigue
depression
general somatic hyperawareness
cog problems
poor sleep
what are the demographics
-young and middle aged women
->55 y/o
-hetergoenous disorder
can be associated with RA; secondary diagnosis
what is the etiology
-unknown but evidence shows central pain processing disorder
-CSF, genetic, functional MRI studies
-biological changes in their brain
everything hurts
talk about substance P
a part of pain processing
-concentration of substance P in cerebrospinal fluid is upregulated
-which speaks to the fact that there are neurochemical changes within the brain of people with this
-IL-8 upregulated too of pro inflammatory markers
what is the medical tx
-not just meds
-exercise, sleep
-start low, go slow
-avoid opiods
-NSAIDs dont work
-change only one med at a time
-target central neurologic mechanisms of FMS

superior to resistance training in terms of pain benefit

mod-intensity resistance training improves funcitonal statu,s tendermessm muscle strength
exercise can improve depressive symptoms
talk about exercise in general
-flexbiilty may be helpful but aerobic and resistnace training more beneficial
-supervised group exercise can ehance adherence
-needs to be tailored to starting fitness level and symptom severity
-aquatic therapy can help wtih dpression/anxiety
