This is a benign tumour of the myometrium

In what demographic are fibroids more common?
Afro-Carribean
What are the risk factors for fibroid?
- Early menarche
- 0 parity
- Obesity
What is the typical presentation of fibroids?
- Menorrhagia
- Longer period
- Dysmenorrhea
- No intermenstrual or post coital bleeding
Also
- Anaemia
- Pressure urinary symptoms
If they ar near vagina you may get dyspeuronia
What would you find on examination?
- Firm mass in the pelvis
- Blood on cusco speculum
- adnexal tenderness
What investigations would you do?
- Pregnancy test
- US
- Bloods - FBC, CA125, BHCG
- Outpatient endometrial biopsy if >40
What are the complications of fibroids?
- Torsion
- Infection
- Cancer
What are the three treatment pathways for fibroids?
- conservative
- medical
- surgical
What is the conservative option?
Explain what it is and do nothing. This is a good option if there is no pain
What are the medical options? (Four)
- Transexmic Acid
- GNRH analogues
- COCP
- Esyma (lowers FSH/LH and reduces susceptibility to progesterone
What are the surgical options for fibroids? (3)
- Myomectomy - Surgical removal
- Hysterectomy
- Uterine artery embolization
What is a myomectomy?
Surgical removal of fibroids
What is uterine artery embolization?
Cutting off blood supply via the uterine artery to reduce flow
The uterus still gets blood via the anastamosis with the ovarian artery
