What is meant by the term PUO
A
pyrexia for greater than 3 weeks with no identified cause after evaluation in hospital for 3 days or greater than 3 out-patient visits

What are the types of PUO
A
- Nosocominal PUO
- immunodeficient PUO
- HIV PUO

What is a nosocomial PUO
A
Patient hospitalized for >48h with no infection at admission.

What is an immunodeficient PUO
A
yrexia in patient with <500 neutrophils/microlitre

What is a HIV PUO
A
Pyrexia in HIV infection lasting 3d as an in-patient or >4wks as an outpatient

How do you investigate PUO
A
- blood tests = FBC, U+E, LFTs, CRP, ESR, Electrophoresis, LDH, CK, ANA, ANCA, Rheumatoid factor, HIV test, malaria smear, interferon gamma elease assay for TB
- Microscopy and culture = blood x3, urine, sputum, stool, CSF
- imaging = CXR, abdominal and pelvis USS, venous doppler

- atrial myxoma
