Accommodation: have patient
focus on point in distance
business card placed where
place 8-10 inches from pt eye and read above thumb
normal accommodation findings
patient brings words into focus quickly
abnormal accommodation findings
words are blurry or patient can’t focus close up
possible CN3 lesion and check other CN3 functions
patient over 40 with abnormal accommodation findings
possible presbyopia (problem with lens)
Convergence: have patient
focus on finger 18” away from patient’s nose
bring finger where during convergence
1-2” of patient’s face between their eyes
repeat 2-3 times
observe what during convergence
smooth convergence of eyes
each pupil to constrict
fx of CN3
Confrontation procedure
in front of patient, pt stare at forehead, hands behind patient slowly bringing wiggling fingers forward one side at a time.
note where patient was able to see hand each time compare to other side
assessed bilaterally, above pt head, at pt eye level, below pt shoulder
peripheral vision is fx of which CN
CNII
normal confrontation findings
patient sees fingers in peripheral vision, bilaterally symmetrical
abnormal confrontation findings
tunnel vision or asymmetrical peripheral vision
state what after confrontation test
would perform or refer out for a more detailed visual fields test if abnormal confrontation test and would perform other tests to check for glaucoma

follow finger with eyes only
Looking at pt
pt look up to left
right eye: SR3
left eye: IO3
pt look left
right eye: LR6
left eye: MR3
pt look down left
right eye: IR3
left eye: SO4
pt look up to right

smooth tracking of eye movements and looked for any lack of movement or incoordination
