What is the volar plate ?
The strongest ligament in the PIP joint. Connect proximal and middle phalanx on the palmar side. Keep the joint from bending too far back.
Function of the radial nerve ?
Goes from the end of the radius to the back of the hand.
Give sensation to the back of the hand from thumb and middle finger just beyond the main knuckle of ring and middle finger.
Function of the ulnar nerve ?
Travel through Guyon’s canal, formed by pisiform and hamate bone.
Give sensation to little finger and half of the ring finger. Control hypothenar and adductor pollicis muscle.
Function of median nerve ?
Travel through carpal tunnel.
Give sensation to thumb, index, middle finger and half of the ring finger. Control thenar muscle.
What are the movement possible at the wrist joint ?
Dorsal flexion = extension
Plantar flexion = flexion
Radial abduction
Ulnar abduction
What are the movement possible at the distal radioulnar joint ?
Pronation, supination
What are the movement possible at the finger joint ?
PIP and DIP :
- flexion and extension
MCP joint digit 2-5 :
- flexion and extension
- abduction and adduction
What are the movement possible at the thumb joints ?
Abduction and adduction
Extension and flexion
Opposition and reposition.
What are bones you can palpate in the hand and wrist ?
Wrist :
- distal section radius and ulna
- styloid process of radius
- distal radioulnar joint
- radiocarpalis joint
Palm :
- scaphoid and lunate bone
- Carpometacarpal and Metacarpophalangeal joint
- metacarpals
Finger :
- phalanges
- proximal and distal interphalangeal joint
What soft tissue can you palpate in the hand and wrist ?
Skin, muscles
Tendon and tendon sheath :
- of flexor : palm of hand, wrist and distal forearm
- palmar fascia
- of the extensor : back of hand
What are the specific test of the hand and wrist ?
De Quervain’s tenosynovitis :
- Eichhoff test and Finkelstein’s test
Suspected carpal tunnel syndrome
Test for rupture of ulnar collateral ligament
Test for suspected fracture of the hand
What is De quervain’s tenosynovitis ?
Suspected tenosynovitis of abductor pollicis longus and extensor pollicis brevis.
Part of the first tendon compartment.
- tendons compartment : 6 tunnel transmitting the long extensor tendons from the forearm into the hand.
What is carpal tunnel syndrome ? What are the signs ?
Symptoms resulting from the compression of median nerve in carpal tunnel. There are several test but they do not help to distinguish it from other conditions causing paraesthesia.
To confirm look at patterns :
- at least symptoms in the first 3 digits
- can radiate down the forearm or on other fingers
What is Kienbock’s disease ?
It’s an avascular necrosis of the lunate bone. Often happen years after a trauma of the wrist.
Swelling, stiffness and weakness of wrist. Decreased motion. Pain over lunate bone.
