Requirements for prescribing buprenorphine
-MDs, NPs, and PAs can prescribe buprenorphine without it if state law allows (before they needed the DATA waiver to prescribe buprenorphine)
-MDs can prescribe buprenorphine once they are registered just as any other drugs (there used to be barriers for prescribing drugs for addiction)
-need to meet some training requirements when registering as a practitioner with the DEA
What are the training requirements for prescribing C-II to C-V drugs?
-8h of training from organizations on OUDs or SUDs for practitioners
Board certification in:
-addiction medicine or addiction psychiatry from the American Board of Medical Specialities, Addiction Medicine, or Osteopathic Association
-or graduating within the last 5 years from medical,
advanced practice nursing, or physician assistant school with 8h on SUD in the curriculum
A pharmacy is allowed to dispense Buprenorphine/naloxone. T/F
Yes.
-As long it is FOR maintenance and detox and meets the purpose of the issue
(buprenorphine mono is only for pregnant, nursing, or those who have hypersensitivity to naloxone
-No other controlled substance can be dispensed legally for maintenance and detox (ex: can’t dispense Methadone for maintenance and detox because it is a C-II)
-> Federal regulation, TN doesn’t say anything about it
Office-Based Opiate Treatment (OBOT) Facility Rules in TN
must comply if:
-if the facility prescribes BUP to >25% of patients or more than 150 patients
some of the rules:
-Must be licensed with TN Dept of Mental Health and Substance Abuse Services (TDMHSAS)
-Counseling required – Narcotics anonymous doesn’t count
What is an Opioid Treatment Program?
-a program engaged in maintenance and/or detoxification treatment with narcotic drugs
-for example: a methadone clinic
Can buprenorphine be dispensed by opioid
treatment programs?
Yes
not limited to methadone, could be any opioid use for maintenance and detox
must follow the OTP rules:
-must treat like any other OTP patient (eg,
behavioral/psychological counseling required)
-must update the DEA registration to dispense C-III
