What are the 3 categories of pharmacies listed in IN
category I- a pharmacy that provides pharmaceutical care to general public by dispensing drug or devices (community)
Category II- a pharmacy that provides inpatient care under a drug order or to an outpatient of the institution under a prescription
Category III- a pharmacy that provides closed door operations that are not open to the general public. May include tran=ditional and non traditional pharmacy functions.
When do pharmacy permits expire
Pharmacy permits expire every two years on december 31st of odd numbered years. Renewal is similar to pharmacist renewal process
What is the primary function of category I pharmacies
They provide drugs or devces to outpatients under a prescription

- an independent contractor exclusively working with hospital

Physical requirements to be eligible for a pharmacy perimir, a pharmacy must do which things
- be stationary (unless specifically registered as mobile location)
- be suffieiently secure and able to detect and deter entry when pharmacy is closed
- Be well lighted and ventilated and otherwise cleanly
- Be equipped with a sink with hot water, a proper sewage outlet and refrigeration
- be of sufficient size to allow for practice of pharmacy
- have other equipment as required by the board to operate in compliance with state and federal law

You cannot practice pharmacy without a pharmacist license and without permit. Inappropriate display of these items or practice without appropriate licensure is grounds for violation
we cant even use words in a title which might cause the public to believe that pharmacy is being practiced (apothecary, pharmacist, drugs etc..)
What must we do when there is a transfer of pharmacy ownership?
Within 10 days of the change of ownership of a pharmacy, an application must be submitted to transfer the ownership
What needs to be done if a new oner of pharmacy moves the pharmacy
An application for a permit for new location must be filed.
If a pharmacy will be closed for 5 days or more, what must be done
permit holder must notify the board of the closure and secure drugs as directed by board
Indiana law about transferring prescriptions
Upon request of a patient, a pharmacy SHALL transfer to another pharmacy a prescription for a patient (including CII) (not FAX)

For how long shall prescriptions be organized, maintained and accessible at pharmacy? Why?
2 yrs. Must be available for inspection to any member of the board or agent of the board
when can we refill a prescription without written, electronically transmitted or oral authorization of a licensed practitioner.
The following conditione=s must be met
1. pharmacist made every reasonable effort to obtain authorization to refill from the prescriber
2. Pharmacist believes that failure to provide refill would be detrimental to pt health
3. Original prescription authorized refills, but all refills were dispensed or prescription expired
4. The prescription was originally filled at pharmacy where the request for refill is received or at an affiliate sharing a real time online database
5. drug can not be PRN, has to be continous and uninterrupted use
6. Pharmacist documents the required refill information, dates, times and attempts to contact prescriber were made, and that pharmacist dispensed without prescriber suthorization.
7. Pharmacist notifies the original prescriber by the rescribers next business day
8. Refill quantity is limited to the most recent fill quantity or a 30 day supply (WHICH EVER IS LESS)
9. max of 1 pharmacist initiated refill for a single prescription every 6 months
10. drug is not a controlled substance
