When assessing a patient’s LOC what are you asking if the patient is oriented to? What else should you note in a patients LOC assessment?
A
When assessing a patient’s LOC ask questions to observe if they are oriented to the current:
Time
Place
Person (Who they are and who you are if they have seen you before)
And sometimes even their current situation.
Also be sure to note if the patient is Awake and Alert, Asleep, or if they are Lethargic and Confused.

When discussing Mental Status what orientations typically are lost first?
A
Time is typically first to be lost, then place.
Rarely will someone lose an orientation to person.
