Pressure Ulcer Stage I
An observable pressure reed alteration of intact skin. Differences may include skin color, skin temp, skin stiffness, or sensation
Pressure Ulcer Stage II
A partial thickness skin loss. Dermis and Epidermis. SUperificial and presents clinically as an abrasion, a blister, or a shallow crater
Pressure Ulcer Stage III
A full-thickness skin loss that involves damage or necrosis of subcutaneous tissue that may go down through underlying fascia. The ulcer presents clinically as a deep crater with or without undermining adjacent tissue.
Pressure Ulcer Stage IV
Full thickness with extensive destruction, tissue necrosis, or damage to muscle/bone/tendon/jt capsule
Wagner Ulcer grade 0
No open lesion but may possess pre-ulcerative lesions, healed ulcers, presence of bony deformity
Wagner Ulcer grade 1
Superficial ulcer not involving the subcutaneous tissue
Wagner Ulcer grade 2
Deep ulcer with penetration through the subcutaneous tissues, potentially exposing bone, tendon, ligament, or jt capsule
Wagner Ulcer grade 3
Deep ulcer with osteitis, abcess or oteomyelitis
Wagner Ulcer grade 4
Gangrene of digit
Wagner Ulcer grade 5
Gangrene of foot requiring disarticulation
Silver sulfadiazine
with/without dressing applied to wound directly painless broad-spectrum effective against yeast
Does not penetrate into eschar!

broad spectrum, non-allergenic, dressing is painless
Poor penetration

Can cause electrolyte imbalance
Removal of dressing is painful
