What is erythema multiforme?
Acute, immune-mediated condition that affects the skin and mucous membranes
Explain the pathogenesis of EM?
It is a type IV hypersensitivity reaction in which basal cells are attacked by T cells

It is a type IV hypersensitivity reaction in which basal cells are attacked by T cells causing inflammation leading to the formation of lesions
What are the types of lesions?
Targetoid lesions (central necrosis surrounded by erythema)
Macules
Papule
Bullae
Vesicles
Types of EM
Minor
Major
What are the features of EM minor?
Usually happens after an infection
Targetoid lesions appear on palms and soles and then spread to the trunk
Only one kind of mucosa is involved
What are the features of EM major?
Severe form
Hemorrhagic crusting of the vermillion zone of lips
2 kinds of mucosa involved
Associated with mycoplasma, herpes simplex
Can be caused by drugs
What are the drugs that can cause EM?
Penicillin
Sulfonamides
Ciprofloxacin
Anticonvulsants (Phenytoin, carbamazepine)
How do we diagnose EM?
Targetoid rash
Hemorrhagic crusting of lips
What will be the treatment of EM?
Maintain hydration
Treat the infection (Css)
Remove the offending agent
Oral Acyclovir, valacyclovir
It is self resolving
