What is clindamycin best used for?
-Best for ADA except for treatment of brain abscesses
-Also covers some gram-positive aerobes
What is clindamycin derived from?
Lincomycin
Clindamycin mechanism of action
Inhibits protein synthesis by binding exclusively to the 50S ribosomal subunit (binds in close proximity to macrolides and synercid)
What is clindamycin antibacterial activity?
Bacteriostatic unless in high concentrations against susceptible organisms
Clindamycin mechanisms of resistance
-Altered target sites (erm)
-NOT active efflux (mef)
Clindamycin gram-positive coverage
-MSSA
-Some CA-MRSA
Clindamycin anaerobe coverage
-Best for ADA
-Not used for C. diff
Clindamycin distribution
Good distribution able to penetrate bone and has minimal CSF penetration
How is clindamycin metabolized
By the liver
Is clindamycin removed by hemodialysis?
No
Clindamycin clinical uses
-Anaerobic infections outside of the CNS such as pulmonary infections
-Skin and soft tissue infections in pen-allergic patients or patients with infections due to CA-MRSA
Clindamycin adverse effects
-GI effects
-Worst inducer of C. diff colitis
