what are the 4 principles of equine wound management?
- assess
- clean
- debride
- provide enviropment for healing

is deep - can see danage to muscle
treatment:
dried blood so needs to be debrided
fresh wound so could be closed - pull muscle together and then subcutaneous (absorbable) and epidermis (non-absorbable)

is there impact to synovial structure as over the joint - (4 joints) , most worried about the tarso-metatarsal joint
treatment:
Radiography
lavage and then leave to close

deep laceration on the carpus - worried about synovial structure and bacteria not sure how long has been left
tendon rupture
Teatment:
lameness assesment
ultrasound
joint tap to see what the synoval fluid looks like (total protein, nucelated cell count and % of WBCs)
radiographic contrast
pressure check - inject saline into the joint far away from wound - if it stays in there then wound not connected if it comes out then wound conected

assesment:
superficial - looks sloughy

treatment:
lavage - with tap water with chlorohexidine, or povodone iodine, then rinse off (as toxic to epithelial cells)
could dress with sliver cream or honey
done bandage as is from a bandage

assesment:
worried about optic nerve
needs to be ultrasound before remove,
referal to hospital
manace response and opthalmascope
Treatment:
after removeal need to flush the wound, povidone iodine
would leave to close, as a deep wound and want to close from the inside out

assment:
worried about pucture of abdomen and depth, has it punctured a organ
puncture through the perionum = infection risk
the external abdominal oblique muscle has been punctured
ultrasound the wound
palpate the wound with sterile glove
do abdominal synthesis - if intestine punctured will be dark coloured with grass
treament:
in intestine not punctured - flushed with povidone iodine
if peritoneum is punctured - peritonitis risk so give systemic antibiotic, flush the abdomen
if intestine is punctured - euthanasia

old non healing wound
assesment-
superfical with excess granulation tissue, = proud flesh, epithelial cells cant close over the wound
treatment:
scalple to get rid of granular tissue making it flat with rest of skin - no need to nerve block as there is no nerves, have bandages ready as it will bleed alot
cut the bottom first and then cut the top if not blood will stop you from seeing what you are doing
puch graft is still doesn’t heal

remove the hook and then lavage dont need to stitch

got leg stuck on fecne no bone damage on xray
worried about the joint
need to do a synovial joint tap
then flush the joint
will close using mattress suture, puncture holes will allow the skin to strech, relive tension and allow for fluid to escape
need to imobilse the leg with cast as the movement will cause pressure on the wound

need to debride and then can close the part that is not under tension but the rest needs to be left open to granulate from inside out
