episcleritis + scleritis Flashcards
what are the features of episcleritis?
- red eye
- classically NOT painful in comparison to scleritis - mild pain or discomfort
- watering and mild photophobia
- visual acuity preserved
what are the differences in injected vessels in episcleritis vs scleritis
injected vessels in episcleritis are mobile when gentle pressure is applied on the sclera
in scleritis vessels are deeper so do not move
what can be used to differentiate scleritis and episcleritis
phenylephrine drops
Phenylephrine blanches the conjunctival and episcleral vessels but not the scleral vessels. If the eye redness improves after phenylephrine a diagnosis of episcleritis can be made
what are the causes of episcleritis
wegner's rheumatoid arthritis SLE vasculitis PMR
how is episcleritis managed?
- conservative
- NSAIDs (ibuprofen)
- artificial tears may be used
what percentage of episcleritis is bilateral
50%
what are the features of scleritis?
- red eye
- classically painful compared to episcleritis - worse on eye movement
- watering and photophobia
- gradual decrease in vision
what are the features of scleritis?
- red eye
- classically painful compared to episcleritis
- watering and photophobia
- gradual decrease in vision
what are the complications of scleritis
scleromalacia - thinning leading to globe perforation
